Provides Physical Therapy to patients in the home setting through the process of assessment, planning, implementation and evaluation. Communicates with patients, families, physicians, RN’s, aides and other therapists to coordinate patient care and evaluate its outcome.An individual that understands and adheres to the Patient’s Bill of Rights.

This position is accountable for provision of occupational therapy service compliance with Agency policies and procedures, applicable healthcare standards and governmental laws and regulations. The occupational therapy functions strives to proactively meet the needs of, and assist the patient/client in the achievement of individualized, defined and measurable outcomes.

Provides diagnostic and therapeutic services to patients exhibiting a speech, language, hearing or swallowing disorder. Maintains systematic records in accordance with professional guidelines. Maintains quality care for all patients through interaction with appropriate personnel in a coordinated, comprehensive, multi-disciplinary approach, and provides assistance where communication barriers exist due to speech language disabilities to enable each individual to reach his/her maximum level of independence. Supports and actively participates in quality improvement program and in-service programs.
